PLEASE NOTE WE CAN NO LONGER TAKE REQUESTS FOR PRESCRIPTIONS BY PHONE

We operate a policy of accepting prescription request forms by written request only. This policy is strictly adhered to and our admin staff are not permitted to accept requests for medication at any time by phone. This policy is in place in keeping with best practice

Requests for repeat prescriptions can be done in 2 ways;

1) Request can be submitted online using the form below

OR

2) Download and print off the Repeat Prescription Form (Download Form) and post to:
Sandycove Surgery, 1 Mount Pleasant, Sandycove Road, Sandycove, Co Dublin

Your doctor will review the prescription request and may require that you make an appointment prior to issuing the prescription if deemed necessary. Prescriptions will be forwarded directly to your pharmacist by secure email.

Important: Please do not leave ordering your prescription to the last minute. Allow a minimum of one week for renewal.

This form is for repeat prescription ordering only. Queries or clinical requests will NOT be seen by admin staff or a doctor.
